(Letter of confirmation of submitting required documentation for re-entry into Japan)
If you are traveling from US, this procedure is only applicable if you have departed from Japan on or before August. 31, 2020.
Please make an appointment first by calling 212-371-8222 ext. 492 (Ext. 492 is for appointments only). You may then apply for a "Re-entry Confirmation Letter" at the Consulate.
Required Documents:
1) Application form https://www.mofa.go.jp/files/100078392.pdf
(Please include your address and phone numbers of where you are currently staying in the US on the blank area of the application form.)
2) Passport which has a valid re-entry or a valid special re-entry permit, and the latest departure stamp.
3) Current residence card from Japan.
4) A photocopy of the passport's "picture page."
5) A photo copy of the valid re-entry permit (If it's a special re-enter permit, the permit is stamped on the back side of the ED card).
6) A photo copy of the latest departure stamp.
7) Photo copies of the front and back sides of the residence card.
Once the "Re-entry Confirmation Letter" is ready, the Consulate will contact the applicant to come and pick it up.
Please note that the "Re-entry Confirmation Letter" is not issued on the same day as the application. We do not have an exact amount of time for processing each case. Some cases may take longer. This process is free of charge.
* Upon re-entry, a COVID-19 test with a negative result is required, and must be conducted within 72 hours of the departure.
